Far Cry 6 Vaas Insanity DLC (Vaas’ Mind) has 18 Vision Locations. Finding all Visions unlocks the So Much For Poetics trophy and achievement.
The Visions during main objectives are semi-missable because they disappear if you run past them or go too far away from them. They would require a new playthrough to respawn. The ones in the open world mostly keep playing infinitely in a loop and can be triggered again if you go too far away from them. They spawn on any Mind Level from Level 1 to 5, all are in fixed positions. They look like ghosts of Vaas and the people he knows, showing memories from his past. Make sure you watch them until the end to make them count. It will show a collectible notification in the right corner when it counted. You can track which ones you found and which ones are missing by going to the Collection Menu and clicking on Visions. Each has its own name and number so you can easily tell which ones you are missing.
They do not need to be collected in one life, it saves across multiple playthroughs. If you die you do not need to find them again.
This guide uses the in-game numbering as seen in the Collection Menu.
Vision 01: Family
On the path to the northernmost Silver Dragon Blade piece, can see a Vaas Illusion sitting on a rock in the red water.
Vision 02: Rebel
During the Birthright Memory objective, after climbing up the plants on the staircase. Walk slowly or you might run past this.
Vision 03: Scripts
During the Birthright Memory objective, immediately after Vision 02 in the same corridor.
Vision 04: Birthright
During the Birthright Memory objective, after 1st phase of Citra boss fight.
Vision 05: Fatal Flaws
During the Birthright Memory objective, after 2nd phase of Citra boss fight.
Vision 06: Temptress
During the Birthright Memory objective, after having defeated Citra boss and picked up the Silver Dragon Blade piece. You will then fall through the floor and land where this Vision takes place, can’t miss it.
Vision 16: Addict
In the north of the map, for completing “The Balance Point” location. There’s a flooded house in the red water. You must dive through the open window underwater, from there open the door, swim up, open the doors. This opens the real entrance doors above water, which are mirrored by the underwater doors. Now you can swim up and enter through the main doors above water. Collect the 3 pieces of Medicine inside the house and place them in the garden house. Then the Vision starts.
Vision 15: Copycat Motherf*ers
Automatically after picking up the Silver Dragon Blade Piece during main objective “Radio Play”, can’t miss it.
Vision 13: Meet Cute
Automatically after picking up the Silver Dragon Blade Piece during main objective “You Hit The Ground”, can’t miss it.
Vision 14: Roleplay
Where the “Checking Out” location is on the map. After watching this Vision you must jump down into the hole where Vaas’ Vision is being thrown. The hole is filled with water so you won’t take any falling damage, then you must go through a cave and watch all Visions, then reach a Hotel and must watch all Visions in each of the hotel rooms. There are a total of 8 Visions as part of this location.
Vision 17: Redefined
During “Checking Out” location, after jumping down the hole where Vaas Visions is being thrown into, when you see Vaas again in the cave always listen to him. Do NOT run past him towards the door of the hotel. Always let his visions finish fully. When you do this, he will eventually do a final talk in front of the hotel door where he raises both his hands, this is the one that counts as the Collectible.
Vision 07: Room 1991
Automatically during “Checking Out” location while exploring the hotel, can’t miss it (1st hotel room).
Vision 08: Room 2003
Automatically during “Checking Out” location while exploring the hotel, can’t miss it (2nd hotel room).
Vision 09: Room 2006
Automatically during “Checking Out” location while exploring the hotel, can’t miss it (3rd hotel room).
Vision 10: Room 2007
Automatically during “Checking Out” location while exploring the hotel, can’t miss it (4th hotel room).
Vision 11: Room 2012
Automatically during “Checking Out” location while exploring the hotel, can’t miss it (5th hotel room).
Vision 12: Final Room
Automatically during “Checking Out” location while exploring the hotel, can’t miss it (6th hotel room).
Vision 18: Warrior
Automatic Vision when triggering the extraction sequence. After getting all 3 Silver Dragon Blade Pieces (Main Objectives – see Visions 06, 13, 15), return to the starting camp and talk to the Vision there. You have to do this to escape Vaas’ Mind so it’s something you’ll get automatically.

#15 can be missed or at least it glitched on me. Stick around until it ends.

As said in the intro, some are semi-missable (particularly the ones automatically triggered from story, which #15 is) if you go too far away from them, then they don’t respawn until next playthrough.
