Collectibles Overview:
Secret #1
After the Power Gauntlet tutorial, go through to the opened doorway, look left, smash through the door and look left again for a Life Sigil.
Secret #2 + Collectible Toy #1 (Imp)
After acquiring the Blue Key (unmissable story objective), shieldbash through the green wall to leave the church. Turn left and go back on the path you took earlier to get to the church, on the right side of the path is a door you can now open with the blue key to find a secret and a toy.
Secret #3 + Weapon Skin #1 (Shotgun – Nightmare)
You will come to a big open area where you get the objective to close 4 demonic portals. Take the path on the left until you find a drop pod that contains the Shredder weapon (picking it up is mandatory for the story). After getting the Shredder, turn around and look to the left, you’ll see an area you can jump down to. Jump down here and approach the collectible, a barrier will raise around it, turn around and shoot the barrels to kill the demons that appear, then you can collect it.
Secret #4 + Collectible Toy #2 (Soldier)
Go back to the same area with the drop pod in the center, proceed past it to another area you can jump down to on the left, go down here to find the secret and a toy.
Secret #5 (Purple Key required)
Once you close the 3rd demonic gate, before proceeding to the 4th by going left to the jump pad, instead go right and around the corner to find the Purple Key. Now return to the starting area where the 1st demonic gate was, here you can unlock a gate using the purple key for the secret and a Life Sigil.
Codex Entry #1 (Doom Slayer)
After closing the demonic gate in the area behind Secret #5, go behind where it was and to the right, climb up around and to the top of the area, you will see the codex entry here.
Secret #6 + Codex Entry #2 (Village of Khalim)
After destroying all 4 demonic gates, proceed to the next area. After the next combat encounter, just past the big fiery wreckage is a climbable wall on the right, climb up here to find the codex entry.
