Resident Evil 9: Requiem contains 21 Antique Coin Locations. Finding at least 17 Antique Coins is required to unlock the 4 upgrades in the Parlor of the Care Center. However, they are not needed for any trophies / achievements.
All of the Antique Coins are found exclusively as Grace in the Care Center. Their only use is to redeem them at the lockers in the Parlor to get 4 upgrades: Hip Pouch (2 extra inventory slots) + Stabilizer (more gun damage and better handling) + Steroids (more maximum health) + Override Manual (plus 50 maximum blood collector capacity). Only 17 coins are needed to purchase all upgrades, the rest are extra and have no use.
This guide is based on Casual & Standard difficulty. The Antique Coins have completely different locations on Insanity difficulty. On Insanity, they are mostly dropped by enemies throughout the Care Center.

Guard Office: In the Safe Room where the typewriter is. Coin is on desk.

In the corridor north from Filing Room.

Bar & Lounge: On the piano.
Antique Coin #4
Bar & Lounge: Dropped by the singing lady enemy in white dress.
Antique Coins #5-#7
Bar & Lounge: On the bar counter behind the enemies. Contains 3 Antique Coins.
Combination (Casual) = Left 10 – Right 80 – Left 30.
Combination (Insanity) = Right 20 – Left 50 – Right 30.

Parlor: After getting the Red Jewel from the Unicorn Trinket Box in Chairman’s Office, go back downstairs through the West Wing and insert the Red Jewel on the Parlor door. Then find this on the Roulette table.

Parlor: Next to the previous coin, behind the counter destroy the porcelain container.

Examination Room: In the corner of the Examination room inside the cabinet you find 3 coins in the safe. You reach this after using the East Wing Keycard
Combination (Casual) = Right 30 – Left 10 – Right 50. Contains 3 Antique Coins.
Combination (Insanity) = Left 50 – Right 30 – Left 90.

Waiting Room: When you enter the Waiting Room, the coin is on the table in front of you.

Waiting Room: Inside the open safe, directly next to the file.

In the corridor outside Waiting Room, on the table before going upstairs.

Directly next to the previous coin, inside the white vase, must destroy the vase to see it.

Conference Room: On the 2nd floor in the Conference Room, it’s on a white desk after entering. First time you go towards this room, the big “Chunk” monster will chase you. You can either defeat it, or come back later after it moved away.

Conference Room: dropped by the singing zombie.

Courtyard: Much later, after escaping the Care Center Basement and getting all 3 Quartz. After opening the quartz door in Central Hall, go down the round staircase, then turn around and it’s on one of the chairs between the stairways.
