The Final Puzzle is a challenge in Resident Evil Requiem. The challenge says “Let the sweet pair hear the voice“. This refers to Emily and Marie’s Doll having to hear the laughing girl in Lead Researcher’s Office. This challenge is entirely optional and isn’t required for any trophies/achievements. It unlocks 20,000 CP extra which can be spent on bonus items.
This puzzle takes place across 2 playthroughs. You can’t complete the steps in a single playthrough. On one playthrough you must get Marie’s doll. Then complete the game with it in your inventory and you will have it permanently for the second playthrough. Then you must enter a combination in the Lead Researcher’s Office Puzzle Box while carrying Emily and having Marie’s Doll in your inventory (after escaping basement). This will finish the puzzle. All steps are done as Grace.
Here’s a step by step guide to solve the final puzzle:
Step 1: Let 115 Bodies drop into the water in “Processing” room of Care Center Basement
In Care Center Basement, after using the Forklift Key to raise the Forklift in the Furnace area, you will go through an area with body bags on conveyor belts. When you reach the end of the moving body bags, you will be in the room titled “Processing“. DON’T interact with the valve yet that drains the water (the one next to a typewriter save). WAIT here and let 115 bodies fall from the conveyor belt into the bloodied water. Be patient and keep count. DON’T leave this area. It will take around 15 minutes real time. It’s the room with the grinder mechanism and a typewriter.
After 115 bodies fell into the water use the valve to drain the water.
Step 2: Finish Grinder Section WITHOUT killing any zombies yourself
In the same room, after interacting with the valve to drain the water, climb down the ladder and the grinder will activate. Let all of the zombies get killed by the grinder. DON’T kill any zombies yourself. Killing any yourself will fail this step.
Step 3: Drain the bloodied water
After the grinder section and grabbing the third Joint Plug, climb back up to the typewriter and go through the vent. You will slide down into a pool of bloodied water. Flip the switch on the wall (before going up elevator) and look at the drained blood pool. You will get the text subtitle at the bottom of the screen “A ton of blood flowed out“. If you get a different subtitle you messed up and must redo the first two steps (wrong body count, killed a zombie in grinder).
Step 4: Flush a Toilet 8 Times with Emily in Restroom
Proceed with the story until Grace escapes the basement with Emily through the elevator and you’re back in the Care Center’s Central Hall. Carry Emily with you to the Restroom in West Wing (accessed from corridor between Kitchen and Dining Room). Flush any toilet in the Restroom 8 times total. Flush the SAME toilet EXACTLY 8 times in a row, not 7 and not 9. There won’t be a confirmation popup or anything.
Step 5: Get Marie’s Doll in Collection & Testing Room
Proceed the story until you went through the Care Center Courtyard (outside), went through the VIP Suite, went through the underground base and found the helicopter keys. The underground base will then start to explode and you are supposed to run back out to the helicopter. Go through the room titled “Collection & Testing” on the map, where a bunch of zombies will attack you. After passing through this area, you enter the backside of the room and the big blood tank in the middle will burst and flood the area with blood. The entire floor will now be filled with a pool of blood. Immediately after the blood tank bursts, you can find the Marie’s Doll on the left side of the area in the blood on the ground. Directly at the metallic stairs that lead out of the room. Be sure to pick up the doll here!
KEEP MARIE’S DOLL IN YOUR INVENTORY AND FINISH THIS PLAYTHROUGH BY RELEASING ELPIS. DON’T DISCARD THE DOLL.
Step 6: Start Second Playthrough
Start a new playthrough, Marie’s doll will carry over.
Step 7: Enter secret code into Lead Researcher’s Office while carrying Emily + Marie’s Doll
Advance story until you exit the Care Center basement where you carry Emily around with you. (After “The Girl” monster’s hand gets severed during elevator ride)
Go to Lead Researcher’s Office puzzle box, in Care Center East Wing, 2nd floor.
Enter this code WHILE HAVING MARIE’S DOLL IN INVENTORY + CARRYING EMILY WITH YOU:

–> A girl will laugh, which is the “voice” you must hear. The “sweet pair” is Marie’s Doll + Emily. If you complete this while holding both Marie’s Doll + Emily you will get a popup that the Final Puzzle has been solved.

It is safe to leave the doll in the item box too. Also no need to start a new playthrough, you can reload an older save prior to leaving the care center and the doll will spawn in the box. I just did this myself on insanity.

so I have to create a new save after I go back up in the care center from the underground place, finish the game getting the doll and then I can load it with the doll spawning in the box?

Just to clarify from my own experience: you do still need to finish the first playthrough. Getting the doll and then immediately loading back a Save at the Care Center with Emily freed does not lock-n the doll to appear in inventory. It’s just that you can load that Save at the Care Center *after* you finished the first playthrough. But you still need to finish that first playthrough.

I did EXACTLY this Step 7, before Emily gets captured. After I cleared all of the enemies on my next playthrough, I went straight to the Lead Researcher’s Office with Marie’s Doll in my inventory and while holding Emily. Managed to complete this challenge as of today!

1. You can’t discard the doll unless I’m mistaken.
2. It can be in either your inventory or the Item Box. It doesn’t matter which.
3. You have to wait 15 minutes which is roughly double 115 bodies falling in. 15 minutes equates to roughly a tonne (1,000 kg) of blood hence “a ton of blood” prompt when draining the blood pool.
4. It appears you don’t actually have to be in that specific area for 15 minutes per se & being in the basement area once the body conveyor belt starts moving is the condition that needs to be met.

I read elsewhere that you can kneecap them etc to slow them down but you just cannot kill them yourself.

When I did the flushing toilets part, I left Emily in the guard room and went to the restroom to flush the toilet. I was able to get Marie’s doll anyway, so I don’t think you NEED to have Emily with you when you flush the toilet.

Nothing works for me. I always get the message lots of blood instead of tons of blood. I’ve tried it about 5 times, I waited 15 minutes, I waited 30 minutes, I counted 115 bodies and I tried it and nothing. Still just the message lots of blood.

I too tried several times in every possible way and always got the message “A lots of blood…”. After eight attempts, I gave up and told myself I’d try again on the next run, but just to be safe, I did the toilet part anyway and saved just before leaving the clinic. When I got to the point where the doll was supposed to be, well, the doll was there. I’m not sure what happened. There’s something I still don’t understand about this.

I want to specify, Body Bags DO NOT count as bodies. You need 115 loose bodies, that’s why it takes so long.

Since PowerPyx has given us the code for the final part, you don’t actually need to finish the basement on the 2nd playthrough. The first time you carry Emily before The Girl kidnaps Emily you can already go to the Lead Researcher’s office to input the code. Worked for me as long as you’re carrying Emily at any point. Btw it’s good to save AFTER you waited 15 Mins+ just so in case something goes wrong (you die like I did because I was trying to be careful not to accidentally kill any zombie lol) you don’t have to wait another 15 minutes. I do believe there’s an autosave in Casual/Standard Modern mode before turning the valve so it’s a non issue for them.
