Mafia The Old Country: Man of Honor DLC has 10 Stunt Jump Locations. Completing all Stunt Jumps unlocks the Stuntman trophy & achievement.
They are all found in Free Ride mode (open world), not during the story. You’ll need to use a car to hit the ramps for them to count as completed. Doing the full jump doesn’t matter, as long as you get the slow motion animation after hitting the ramp. You’ll be able to keep track of your Stunt Jumps in the bottom left corner of the map.
The Stunt Jumps require purchasing the Man of Honor DLC.

In the middle of where the roads connect.

Next to the small bridge.
Stunt Jump #3
Along the cliff while following the train tracks.
Stunt Jump #4
Next to the small bridge by the canal.
Stunt Jump #5
Along the side of the road, by the river.
Stunt Jump #6
On the side of the road.
Stunt Jump #7
On the road, near one of the buildings.
Stunt Jump #8
In between where the 2 roads connect.
Stunt Jump #9
In the middle of the patch of purple flowers.
Stunt Jump #10
Along the road, in between the pillars.
