Beast of Reincarnation contains 22 Document locations. Collecting all Documents is required for the trophy/achievement Know-It-All for reading every document. Documents are found throughout the game by interacting with specific books in the environment, as well as automatically obtained during story progression.
Nothing is missable, you can still find everything after the story in free-roam before entering New Game+.
When you are near a document it will be marked on the map with a “DOCUMENT” icon. They stay marked on the map after collecting them and the icon will show a checkmark, which makes it easy to know what you’ve found. The map icons reset in New Game+, so it’s best to grab everything in a single playthrough.
Collectibles Overview:
Documents are listed in the order that they appear in the Database > Document Log menu, which keeps track of which documents you have collected in each region and allows you to read them again. The exception to this is the final unnamed document collected in The Capital, which isn’t listed but is still required.

Southeast of River Fork campsite across the river, inside the building.
Document #2: A Golem’s Worst Enemy
West of Quarry Path campsite in the Quarry area, on the ground to the left after riding the elevator.
Document #3: Arsonist’s Ramblings
In the building south of Cliffside Village: Interior.
Document #4: Take the Yellow Key
Deal with all enemies inside Subcity 02 and after defeating them the door in the room behind the two malefacts will open, where this is on a table.
Ancient Civilization Zone
Document #5: Yearning for the Surface
Northwest of Reservoir: Interior campsite, on the ground next to the control panel used to open the gate during the story.

This document is automatically obtained when you interact with a Dragon Vein Shrine for the first time. The Great Wall Terraces

North of Mountain Village Entrance campsite on a Golem body.
Document #8: Diary of a Pilgrim
In the final room after Subcity of Flowers: Interior campsite that opens after defeating the Elite Golem.

In the same room as Maze of Sin Interior campsite, on a golem body.
Pilgrim’s Zone
Document #10: Light the Pyres
Northwest of Belfry Entrance campsite, inside the Belfry area on the lower floor, on a table in front of the large statue on the west side of the room.
Matsukawa Zone
Document #11: Iron Body
South of Library Hall campsite, grapple to the second central platform then follow the path to the southwest to a room with this document.
Document #12: Memories…
Northeast of Mausoleum campsite, in the room at the top of the stairs.
Document #13: Vestiges in the Clouds
North of Library Hall campsite, grapple to the second central platform then follow the path to the west to a room with this document.
Document #14: Research at the Capital
Southeast of Upper Matsukawa Colony (Front) campsite, grapple to the first central platform then go to the east side where there is a path leading to a room in the east wall. Inside the room on the ground is the document.
Document #15: Deterioration of the Soul
Southeast of Hall of Silence campsite, enter the room and interact with the door on the lower floor to trigger three waves of enemies. Defeat the waves to open the door, then enter to find the document.
Document #16: Insufficient
Southwest of Library Hall campsite, on the upper level in the southwest corner of the room behind the Elite Golem.
Document #17: The Doctor’s Legacy
South of Upper Matsukawa Colony (Front) go through the door on the central pillar and then take the elevator down to the lower area. After taking the elevator down to the lower level it is on a table in the first room.
Document #18: Dreams Just Out of Reach
South of Upper Matsukawa Colony (Front) go through the door on the central pillar and then take the elevator down to the lower area. After taking the elevator down to the lower level it is the glowing console you can interact with in the second room.
Document #19: Signs of Life
Southeast of Matsukawa Colony Central Area on the console in the same room.
Document #20: Beneath the Colony
South of Upper Matsukawa Colony (Front) go through the door on the central pillar and then take the elevator down to the lower area. After taking the elevator down to the lower level it is on a table in the second room.
Giant Tree Zone
Document #21: Doctor S’s Report
This document is automatically obtained during story progression, after defeating the second Corvus boss.
The Capital
Document #22: Unnamed Document
This document is obtained when picking up the Blade Camelia, which is found after defeating Kunai and Kirin boss inside the Cleanse Walker opposite Emma’s bed.
