Introduction
Welcome to the Mafia The Old Country Trophy Guide (aka Mafia 4)! This game has a similar structure to Mafia 1 Remake, it has a linear story mode and an open world free roam mode. The story takes place in the open world but you are forced to stay within the bounds of story objectives. There are 14 chapters and a Prologue. The story takes around 10-12 hours to complete. There’s also a Free-Roam (Free Drive) mode, unlocked during Chapter 3 and accessed via the Carcyclopedia Menu in the Title Screen. This is where you’ll spend a few hours after the story to search for collectibles. Some collectibles are exclusive to story chapters but the majority is available in Free-Roam. They get marked on the map when going near them, except Hidden Foxes. All progress is shared between both modes. It’s best to focus on the story first to unlock all game features, afterward explore in Free-Roam for remaining collectibles. Nothing is missable thanks to Chapter Replay.
Step 1: Finish Story on Hard Difficulty + Chapter-Specific Trophies + Combat Trophies + Story Collectibles
Hard difficulty is available from the start. To save time it’s best to play on hard from the beginning. Hard difficulty in this game isn’t too challenging. If you’re near death you will automatically regain a healthbar after a few seconds, allowing you to take another hit. Resources are plentiful when looting enemies.
As you play through the story, get all of the Chapter-specific trophies and Story-exclusive Collectibles.
The trophy relevant Collectibles are the 50 Hidden Foxes and the 3 types under the Collections Menu: 23 Photographs, 20 Saints’ Cards, 50 Newspapers. Notes listed under the Journal Menu, Charms, and Trinacria Artifacts aren’t required. Some collectibles only spawn during specific chapters. Don’t worry if you miss one, everything can still be found after the story through Chapter Replay, and most items can also be found through Free-Roam mode.
Additionally, farm any of the combat trophies via checkpoint restart when you have the correct weapon. They are all dropped by enemies. An easy farming location is Chapter 11, enemies drop all types of weapons there.

Step 2: Collectible Cleanup in Free-Roam
After the story, enter Free-Roam mode to clean up collectibles. The collectibles get marked on the map when you go near them. The ones that are story-exclusive will have a special yellow map icon. When you scroll over the icon they specify in what chapter they are found. This makes it easy to track down your missing ones. If you need to replay a Chapter, you can quit out as soon as you found the collectible, it saves instantly and carries over to Free-Roam. You can quickly jump between Chapters to go for only your missing collectibles and then quit out.
Free-Roam is accessed through Title Screen > Carcyclopedia > hold  to Drive. The following collectibles are needed for

Step 3: Miscellaneous Cleanup
All you might have left are some miscellaneous trophies, such as driving at 120km/h for 15 seconds, doing a 40 meter jump, or buying all vehicles. If you need money, you can farm it quickly at the start of Chapter 7. In the room where the chapter starts are two blue chests, open them to get money, restart checkpoint and repeat. The money carries over to Free-Roam mode and all other Chapters. This lets you quickly buy all items you want.

If you’ve already done a playthrough on an easier setting you’ll want to choose “New Game” in the title screen. Hard difficulty is available from the beginning, it’s recommended to do the first playthrough on hard as to not have to do a second playthrough. Note that the trophy can’t be cheated by replaying just the final chapter on hard, nor will it unlock when you switch from easy to hard before the last boss. It has to be the whole game. Hard difficulty isn’t too hard at all. If you are near death you will automatically regenerate one health segment after a few seconds, allowing you to take another hit. There is also plenty of ammo and healing items from looting enemies. Stay in cover, go for headshots when possible. Especially the Pump-Action Shotgun and Praecione Rifle are good to kill enemies in one hit. | ||

This is pretty much an automatic story trophy because to win the race for the story you’ll do it in under 8 minutes anyway. The time limit is very generous, it can be done in under 6:15 if using the shortcuts. There’s no need to drive perfectly. If you have a big crash you can restart the checkpoint. There are multiple checkpoints throughout the race so you don’t need to replay the entire race if you restart. If needed you can also replay this via chapter select. | ||

Instead of going after him, ignore the enemies and go to the left of the area where 2 cars are parked. Destroy the left car that’s parked further back, that’s Caccini’s car. With it destroyed he can’t escape and will get stuck there. Now kill the enemies (or run past them) and Caccini will stand in front of his destroyed car, instead of him driving away. | The trophy description is a bit wrong/misleading for this. There are 3 TYPES of knives: Scannaturi, Rasolu, Stiletto. You only need to use each knife class special combat skill 5 times for ONE KNIFE of EACH CLASS. Stilettos don’t have a combat skill, so you only need 2 knives total: Scannaturi + Rasolu. At weapon boxes where you equip knives it shows the “Special Action” under “Durability” (NOT the Perks). Using the special skill consumes knife sharpness, so make sure you have sharpness slots remaining. It can be farmed via checkpoint restart. An easy spot is Chapter 5 – Objective: Infiltrate Ludovici’s Villa. Where you talk to Tino before infiltrating the villa, there’s a knife box that lets you change knife types. Use the knife special ability on 5 enemies, restart checkpoint, switch knife type and repeat. You can also buy all knife types at the weapons box there, can use the cheapest knives that cost 200 money.

Note: during the chase at Ludovici’s villa, you will get the task to get Ludovici alive, but you can’t do anything wrong here. For the main story objective you will always capture him alive and bring him to Tino (if Ludovici dies during the chase it results in a game over). | *MISSABLE* – If you collect all notes containing safe combinations they will stay in your inventory even when replaying chapters. The game treats this as already knowing the combination and then it won’t unlock via Chapter Replay. This is best done in Chapter 3, at the Citrus Farm during main objective “Collect the money”. Here the story makes you open a safe for the first time. Normally, you’d get the safe code by collecting the notes in the same room where the safe is. However, for this trophy DON’T collect the text notes and enter the combination directly, it is: 10-20-30 for this safe. It’s always the same combination. When you open the safe without having read the note that contains the combination, you’ll earn the trophy. It doesn’t seem to unlock through chapter replay in this section if you already collected the note previously. If you already opened this safe, there are many other safes throughout the game. You can make a PS+ backup when reaching a safe, then read the notes to get the code, redownload your PS+ backup (thus removing the note) and open the safe directly. | For this you must buy all 7 purchsable cars (other non-purchasables not required). All cars will be available for purchase after Chapter 7. You can then buy them at any garage either during the story or in free-roam mode. Buying all of them costs 11,000 money total. If you need more money, start Chapter 7 and in the first room where the chapter starts are 2 blue chests. Open them, restart checkpoint, repeat. They give around 100 money each (more if you equip a Charm/Bead that gives more money). You can quit the chapter immediately when you got enough money and it carries over to free-roam mode. The garages where you buy vehicles are marked as fast travel points on the map, for example at Villa Torrisi and Pasquale’s Store. Purchasable Cars List (11,000 money total):

| This is best done in free-roam but can also be earned during the story. First you’ll need to unlock a fast enough car that reaches top speeds over 120 km/h. During Chapter 7 you automatically unlock the “Carozella Mark 2” car for free, it reaches 162 km/h. Now go into free roam mode and drive on a long straight road, maintain high speed for 15+ seconds to pop the trophy. For example, from Porto Almaro fast travel point take the road leading northeast. Alternatively, you can also earn it during the Chapter 7 race event. Shortly after the start of the race, after exiting the town, there’s a long relatively straight stretch of road, if you drive perfectly without crashing or breaking you can earn the trophy there. | ||

I love how the devs basically trolled everyone with the racing trophy, making people sweat bullets only for the race to be super simple and include checkpoints in case you crash.

I tested it today on patch 1.003.000 and didn’t work. I played story on easy up until the end. Switched to hard before the last boss, no hard trophy popped. Replayed whole last chapter via Chapter Replay on hard, it didn’t unlock again.
Maybe it worked on old patch, but not anymore.

Could use extra confirmation if Salvatore’s Apprentice still pops via Chapter Replay if already having collected the notes with safe combination in a previous run.
The notes respawn, but stay in your inventory. The game seems to treat this as already knowing the combination. I tried it on the first safe in Chapter 3, had collected the note previously and it didn’t give the trophy on chapter replay. So this is potentially missable. Luckily there are multiple safes throughout the game to get another chance.

Thank you for the trophy guide. I want to play Mafia: The Old Country eventually considering the game seems to be good and the Platinum trophy difficulty is reasonable, but since it’s a shorter game, I’ll wait for it to be cheaper digitally or if it’s added to PS Plus.

The fox statue in the weapon shop seems to be bugged. It costs $5000 to pick up but if you have less than that, it’ll take all your money anyway and make the statue disappear but the game doesn’t register you having collected it. Restarting a checkpoint doesn’t fix it. I think my save is broken now.

Same here… I hit reload last checkpoint as soon after I interacted with less than 5k, and bought the fox when i had enough money… but I can’t get the trophy to pop even if I have collected all 50 foxes.

Tip: man of honor trophy you can obtain also in the chapter 13 (at the church), chapter 5 and chapter 13 is the two possibilities.

The Collector trophy is currently glitched ! Got every saint’s cards, photo and newspapers but the trophy didn’t pop.

It requires finding all foxes too. Getting just the 3 types of Collection items isn’t enough for The Collector.

In my opinion it was totally manageable (maybe even a bit too easy for my taste). There are plenty of checkpoints, and I felt like the stealth was pretty forgiving too, so I don’t think there’s anything to worry about.

Only few first chapters. With right talismans later it’s become easier.
Also, biggest part of the game should be done through stealth and enemies are dumb like stones.

Not really, as long as you take it easy on the shootings you’ll be fine. You usually get bandages as soon as you use them too.

Great guide as always. I cant manage Secret Assassin trophy. I killed more than 20 but nothing popped. I almost kill them with a behind single circle button takedown. And after a while I even tried to grab them first and then killed with a knife. It did not worked as well. I looked for a video but couldnt find it. We all played 15 ass. creed games and i think silent kill shouldt be that hard Is there knife weapon trick or bug. Thanx in advance.

I got the same problem, I did plenty more stealth takedowns already but it just won’t pop. Somebody got the same problem and found a solution?

What I did at first was grappled the oponent and than pressed circle to execute him. This does not count. Instead, when you approach the enemies from the back, press circle immediately and Enzo will do the execution immediately. Do this 20 times and you will get the trophy.

Think its buggy. I’ve done 30 and nothing so far. I am pretty sure I’ve gotten much more than 20 weapon kills as well before those achievements finally popped.
I’m wondering if for some reason checkpoint reload doesn’t work for knife takedowns? Definitely works for weapons, perhaps knife it doesn’t save your progress? Just a theory.
May just play the rest of the game without re-loading checkpoint for them and seeing if it pops that way.

Update: Popped after another 7 kills. Just keep going, it’ll pop eventually even if it takes more than 20.

Secret Assassin trophy popped finally, I’ve repeated chp04 on easy difficulty, and just did stealth takedowns with circle button and do reload last checkpoint as powerpyx mentioned earlier.
Tips:-
• Use Forzuni knife to loot during stealth takedowns.
• Activate charms (San Francesco D’assisi , Fantasma , Astuto and ingegnoso) to increase whole stealth performance.

The fox count is stuck 49/50 for me. I think the one at Pasquale store glitched because I didn’t had the money and it disappeared when I touched it. On the map it’s shown as collected but not registered

I added a bug warning to the trophy guide overview, collectible guide page, and fox pages about it.
I tested it and wasn’t able to replicate it (for me the fox just remained grayed out until I had enough money to buy it), but there are many comments about it so it seems to be a real bug. I’d recommend rechecking all fox icons on the map though as it may also be another fox missing. Some people in comments mentioned Fox #6 was their missing one after 49/50.

For anyone reading this, I’d advise leaving the fox alone and grinding to 17K money with the Chapter 7 money farm immediately when you get to the start of the chapter to set you up with enough cash for all cars (11K), the final mysterious fox (5K) and car customisations (1K). Just put a podcast on for about an hour, makes it stress free.

There’s something weird going on with the True Soldato trophy. For me, it didn’t pop up after finishing the story on hard difficulty. I started the new game and got the trophy in the end of chapter 2. My theory is if you ever change the difficulty settings, just to see the description for example, it saves instantly without any warning, and the chapter you are on at this moment doesn’t count as completed on hard.

I only switched to hard mode in the third chapter. I finished the game and the trophy didn’t drop. I replayed all the missions on hard mode and it didn’t drop either. I started a new game on hard mode and, to my surprise, at the end of the chapter, the trophy for finishing on hard mode dropped.

Two things:
1) Trail of Destruction is not automatic. On Hard difficulty you can miss this. I thought I counted 10 car destructions exactly, but on my original playthrough it actually didn’t give me the achievements. I did chapter replay on easy (DO THIS AFTER GETTING TRUE SOLDATO) and I got this easily with 14-15 car eliminations.
2) For Worthy Opponent, there are checkpoints at 1/3 damage, and 2/3 damage. Additionally, fif he comes at you, you can dodge as per the guide or heavy slash him mid-animation with RB and you’ll always hit him before he hits you. I only tested this in phase 3/3 but I got the final 1/3 of his health down 2nd time with this method after 10+ tries per phase/checkpoint 1 and 2. Worth a go if you’re struggling.

Can you do hard mode using Chapter select or do you need to start a New Game and make the difficulty selection there? The reason I ask I’d like to have access to all my weapons and charms I’ve unlocked.

Bombas Away trophy is bugged for me. Killed more than a 100 enemies with grenades so far. Still won’t unlock

Same for me. Tried chapter 4,5 and 11 many times and it just wouldn’t pop. Did everything else and only had that last trophy for plat. What worked for me is i backed up and deleted my save file and Uninstalled the game. Reinstalled and just rushed the game on easy until ch 4. where I tried killing everyone with grenades. Trophy popped when I blew up the two guys at the beginning of the bandit camp holding Isabella’s ” future husband” Good luck!

Just thought I’d add that the guide for collectibles that are only found in chapters is missing Mystery Fox #13 which is in Chapter 13 at the church.

That fox is available in free roam at any time, it’s not exclusive to Chapter 13.
