Surgical Extraction is a Main Story Quest (Operation) in Far Cry 6. This walkthrough will guide you through all objectives of the Surgical Extraction main mission.

Mission Info: Talia’s going to die unless she gets antibiotics. The army is smart enough to keep some at their checkpoints. Bring them to me.

- Infiltrate the villa
- Search Sergio’s office
- Reach the marina

Starting Location: Surgical Extraction
Surgical Extraction is an operation available in the Valle De Oro region and starts automatically via a phone call after completing “Intervention“.
Recover the antibiotics
You will need to assault one of three checkpoints in the north of Barrial to recover antibiotics. Head to one marked on the map and you will find antibiotics inside the checkpoint building. If there’s no antibiotics there, check one of the other two checkpoints.
Return the drugs to Matias
Head to the mission marker at San Rafael Hospice to return the drugs to Matias.
Locate and interrogate Sergio Estevez
Head to the mission marker at Orquidea Villa.
Infiltrate the villa
Fight your way through the enemies or stealth through and head inside the villa.
Search Sergio’s office
Once inside, head upstairs to Sergio’s office. Interact with his answering machine and computer to get the information.
Reach the marina
Head to the marina to the east.
Search the marina for Sergio
Go to the waterfront of the marina to find Sergio.
Catch up to Sergio
Jump on a quadbike and chase after Sergio. You can’t kill him, so you will just need to chase him and damage his car. Eventually you’ll be on a beach with some red seamines around, shoot one of these to stop him.
This finishes Surgical Extraction operation in Far Cry 6.

Playing on PS4 my mission then goes to a black screen and I can’t do anything. I can go into the menu and change my load out. But if I go back to actually run around all I get is a black screen and some background noises.

I have exactly the same issue, cannot fast travel to a different part of the map either as it says I’m in a mission. I lodged a fault ticket with ubisoft but they were useless. Anyone else have any ideas?

Sergio keeps dying, I’m not even trying to kill him, he either crashes his car or drives into the land mines on the beach…. So dumb…
