Kingdom Come Deliverance 2 The Royal Physician Walkthrough

The Royal Physician is the 1st DLC quest in the Mysteria Ecclesiae expansion of Kingdom Come Deliverance 2. This walkthrough will guide you through all objectives of the The Royal Physician DLC quest.

  • Starting Location: Kuttenberg > Suchdol
  • Quest Giver: Peter of Pisek
  • Requirement: Own Mysteria Ecclesiae DLC
  • Reward: Gain entry to Sedletz Monastery
  • Quest Type: Side Quest
  • Quest Description: Lord Pisek tasked me with going to see Master Albich of Uniczow – a renowned physician who now lives at the house of the Tarmarian apothecary in Kuttenberg. Albich is planning a visit to the Sedletz Monastery and Lord Pisek is looking for someone to accompany him.
  • DLC Required: Mysteria Ecclesiae

Objectives:

  • Speak to Albich of Uniczow
  • Speak to Albich about the departure for the monastery

Starting Location: The Royal Physician

This is the first quest in the Mysteria Ecclesiae DLC. It starts in Suchdol, in the west of Kuttenberg region. A purple icon will appear in Suchdol castle once you purchased the DLC and reached the second day of Main Quest 13: The Sword and the Quill. Go up the stairs into the castle, enter the banquet hall to the right, and talk to Peter of Pisek who will sit on a chair at the table. Before starting the DLC you should keep a permanent Saviour Schnapps save. This allows you to replay the DLC at any time without needing to replay the main game story.

If you’ve already completed the main game story, you will find Peter of Pisek in Kuttenberg instead. He might also travel on the road between Suchdol and Kuttenberg, but he’ll always be marked by the purple icon.

 

What dialogues you pick here doesn’t matter. There is one optional skill check but there’s no consequence if you fail it or pick the other option. To gain reputation pick these: “I’d be glad to help (Reputation+)” > “I think I know who it is (confirmed to succeed at 24 Scholarship skill, Reputation+)” > “That sounds serious” > “Sounds like a good idea” > exhaust all questions > “Alright”.

Afterward, Lord Pisek gives you a Letter of Introduction that you must bring to Albich.

Speak to Albich of Uniczow

Go to the apothecary in Kuttenberg city. It will be marked on the map. Talk to the trader behind the counter inside the apothecary.

He will tell you that Albich went to treat a neighbor’s maid 2 houses to the east. The map marker will be updated.

In front of the house you find Matthew Ram of Tseyle and his bodyguard banging on the door. Approach them and in the bottom right the “Investigate” prompt will pop up. Press :l1: / :lb: / ALT to investigate.

This updates the map marker again to the south entrance of the house. Approach from the street to the south and enter the door through the backyard.

Note: the next set of dialogues has a skill check. Wearing a high charisma outfit (20+) is recommended to succeed.

Go upstairs to find Albich treating a sick maid.

It’s best to wait until he steps away from the maid, if you talk while he’s treating the maid you will lose reputation for interrupting him. After he stands up and faces you, you won’t lose reputation. Then to gain reputation say: “I’m sorry” > “Here’s Lord Pisek’s Letter” > “They say you want to see the King…”.

The two men who were banging on the front door will appear. You can either win any of the 3 skill checks to resolve the situation peacefully, or opt to fight them. If you put on your high charisma outfit, pick “Violence is not the answer” (confirmed to succeed at 20 Charisma). Solving the situation peacefully without threatening to fight them gains you reputation with Albich.

  • Violence is not the answer = confirmed to succeed at 20 Charisma (Reputation+) – the two men leave peacefully
  • You want a fight? = confirmed to succeed at 18 Dread (2x Reputation-, not recommended) – the two men leave peacefully
  • Albich treats the king! = confirmed to succeed at 24 Scholarship (Reputation+) – the two men leave peacefully
  • Enough! (fight) = the two men will attack you, but Albich won’t like it

Speak to Albich about the departure for the monastery

After dealing with the men, Albich is ready to depart to Sedletz monastery immediately. If you aren’t ready, you can tell him you need more time to prepare. He can then be found at the apothecary, usually sitting on the bench at the front entrance during day. Finish up any pending time-sensitive quests before departing to the monastery.

POINT OF NO RETURN:

Once you tell Albich you’re ready, you will be locked into the Sedletz monastery until you complete the DLC storyline! You won’t be able to return to the open world until the DLC is finished. Afterward you can continue working on other quests. The warning screen tells you not to bring any stolen items but it’s actually fine to bring them and the guards won’t search you. You should stock up on Saviour Schnapps (bought from inns) so you can keep a manual save at the start of each DLC quest, if you need to replay anything for trophies/achievements.

Recommended items to bring:

  • 10 Saviour Schnapps to keep a save from every quest, or use the trick to make an Exit Save > use Saviour Schnapps > reload Exit Save to get Saviour Schnapps back.
  • 20 Lockpicks (but the walkthrough will point out where to get keys to locked doors so lockpicking is optional).
  • Having high enough Thievery skill to pick hard locks is beneficial.
  • A high charisma outfit, if you don’t already have one, buy fine clothes from tailors in Kuttenberg (25+ Charisma recommended to win skillchecks).
  • Some Bandages and Merigold Decoctions for healing
  • A torch

Once you agree to go to the monastery, you can ask some more questions, then say “Let’s go then” to get a cutscene. You will arrive inside the monastery and talk to Abbot Jan III.

To gain reputation say: “Does it look like the plague?” > “I’m sorry” (Reputation+) > “Who are the sick?” > “How could they all have got infected?” (Reputation+ if success, confirmed at 24 Scholarship) > “What now, Albich?” > “Alright”.

 

It immediately starts the next quest Anamnesis.

This finishes The Royal Physician quest in Kingdom Come Deliverance 2: Mysteria Ecclesiae DLC.
